OFVA for Participation Doctor
The OFVA is a non-profit organization which was founded in 2002 with a mission to provide eye care to children. Now that there are so many resources that enable children to access care, we are expanding our services to adults as well
- Patients/applicants submit their info to the OFVA via the portals at www.ofva.org.
- Our volunteers review the information provided by the applicant regarding need and income eligibility.
- The applicant’s contact information is provided to you, the OFVA participating provider. Your staff verifies whether the individual qualifies for a medically- necessary eye exam (for example diabetic eye exam) or has other insurance to cover the services.
- Your staff schedules the patient, and you provide the eye exam (billed to insurance or free-of-charge, fits and orders the patient’s glasses through the LEAP lab or other charitable program you utilize for eyewear and later dispenses the glasses to the patient. You will be provided a set of LEAP lab frames to keep at your clinic for all future OFVA orders.
- The OFVA covers the cost of the eyewear for the patient if ordered throuh the LEAP lab.
